Social Prescribing Service

Social prescribing is a way for local agencies to refer people to a link worker. Link workers give people time, focusing on ‘what matters to me’ and taking a holistic approach to people’s health and wellbeing. They connect people to community groups and statutory services for practical and emotional support.

Link workers also support existing community groups to be accessible and sustainable, and help people to start new groups, working collaboratively with all local partners.

Social prescribing works for a wide range of people, including people:

  • with one or more long-term conditions
  • who need support with their mental health
  • who are lonely or isolated
  • who have complex social needs which affect their wellbeing.

Social Prescribing Link Workers help to reduce health inequalities by supporting patient to unpick the complex issues that are affecting their wellbeing.

Physiotherapy

Our First Contact Practitioners (FCP) are physiotherapists who are experts in the assessment of MSK problems. The FCPs work as part of the GP practice team and are available to see instead of your GP, if you have a muscle, joint or soft tissue problem.

Our FCPs are highly skilled, and can offer services such as joint and soft tissue injections, prescribing medications, and referral for investigations to help diagnose the problem.

You can book an appointment with them on a Friday, via Reception or telephone us on 01244 351000.

We also have a physiotherapy practitioner who has a clinic on a Tuesday who can provide diagnosis, self-management advice, exercises as well as onward referrals and investigations such as x rays and blood tests. At your appointment the physiotherapist will assess your symptoms and discuss your treatment options. They can answer any questions and help you decide how best to manage your condition.
