General Practice Extended Hours Service

This service provides patients with GP and Nurse appointments, blood tests and physio assessments outside of core general practice hours.  Monday to Friday 6:30 PM - 9:30 PM, Saturdays & Sundays 9:00 AM - 6:00 PM and Bank Holidays 9:00 AM - 2:00 PM.  To make an appointment call 0300 123 7743.  The phone line is operational from 8:00 AM - 8:00 PM seven days a week.  For further information and areas this service is available select Extended Hours from our side menu.

 

Nurses

The Practice Nurses hold daily clinic sessions between 8.30am and 12.00 noon and 2.00pm and 5-00 pm. Appointments can be made for smoking cessation, chronic disease management and advice, dressings, suture removal, ear syringing, injections, blood pressure checks, holiday vaccinations, cervical smears, well woman and well man checks, health screening and advice on lifestyle. New patients to the practice over the age of 5 years are offered a health check when they register.

Health Practitioners

Sister Louise Hammond (f)

Our Nurse Practitioner Louise Hammond is available Monday, Tuesday and Wednesday. She has undertaken extra medical training to work as a Nurse Practitioner. This means she can prescribe and make clincal referrals.  She can help in many areas, instead of seeing the GP such as; women's health problems, contraceptive advice, children's illnesses, musculoskeletal problems, acute chest problems, minor illnesses, minor injuries and skin conditions.

Healthcare Assistants

The healthcare assistant's role is to assist the GP and nurses to provide care. They can do a number of tasks, such as, taking blood, ECGs, BPs, Health Checks. We have healthcare clinics every day of the week at Fountains.
