Registering As A New Patient

If you wish to register at the Practice and are within the practice boundary, ask at reception for a new patient registration form. You will be asked to complete this and a patient questionnaire. If you wish an appointment can be made for a new patient health check, which will be carried out by one of our nurses.

Registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Alternatively you may print off a registration form and new patient medical questionnaire, fill them both out and bring with you on your first visit to the practice.

Temporary Resident

A patient can be offered to register as a temporary patient if they are resident in the practice area for more than 24 hours but less than 3 months.

To register as a temporary patient simply contact any local practice you wish to use. The surgery may accept you as a temporary patient, depending upon availability of appointments. We aim to offer you emergency treatment if you require this. A doctor may triage your condition on the phone before offering an appointment in the surgery.

You cannot register as a temporary patient at a practice in the same town or area where you are already registered.
